Immunoadsorption plasmapheresis (IAPP) has been widely applied for the treatment of neuroimmunological disorders, including myasthenia gravis (MG). Guillain-Barre syndrome (GBS), chronic iuflammatory demyelinating polyneuropathy (CIDP). and multiple sclerosis (MS). In antibody-mediated autoimmune disease. such as MG, IAPP seems more rational and efficiently removes causative autoantibodies without need for replacement fluid. On the other hand, in T cell mediated neuroimmunological disease, the mechanism of the action of IAPP has not been fully elucidated. Although the efficacy of plasma exchange has been demonstrated by randomized controlled trials in GBS, CIDP, and an acute type of MS, very few controlled trials' using IAPP have been carried out. It may be very difficult to perform randomized controlled trials in a rare neuroimmunological disease. Therefore, it is important to clarify the effects of IAPP on the humoral and cellular immune system. Randomized controlled trials to determine the effect of IAPP in a couple of neuroimmunological diseases would support evidence of the efficacy and establish safer therapeutic apheresis.
