Plasmapheresis in a Patient with Idiopathic Chronic Cold Agglutinin Disease and Acute Heart Failure

An experience of plasmapheresis (PA) in a patient with idiopathic chronic cold agglutinin disease and acute heart failure is described. The patient was an 89-yaer-old man. He had repeated acrocyanosis, Raynaud's phenomenon and coldness of the limbs for six years. These symptoms had been resolved by retaining body warmth in every previous admission. In this admission, however, the cold agglutinin (CA) titer increased up to 1:131,072. He was in a state of acute heart failure accompanied with severe abnormal electrocardiogram (ECG). His symptoms could not be corrected by warming or chemical agents. Hence, four times of PA were performed using a CS3000 cell separator. A total of the ECG abnormality were observed. The titler of CA decreased transiently. These results suggests that the PA made recovery from the acute heart failure in chronic CA disease possible.
