To study the behavior of regenerative nerve in bony defect histologically, the porous hydroxyapatite granules (HAP) were implanted into artificial periodontal defects made in twenty-one mongrel dogs. Following a healing period of 3-months after extraction of bilateral premolars in mandible, 3-wall bony defects were created surgically on the premolars and molars bilaterally. The roots were planed and reference notches were placed on the surface of the root to identify the alveolar crest and apical extent of the defects. Implant sites were filled with HAP and unfilled sites served as controls. Dogs were sacrificed after 1-day, 3-day, 1-week, 2-week, 1-month, 3-month and 6-month postoperatively. The results and conclusions were as follows : 1) A long junctional epithelium occurred in both implant sites and control sites. 2) The control sites demonstrated complete healing with new bone formation but the central portion of the new alveolar ridge was concave because of the proliferation of lamina propria of gingiva. In contrast, healing of the implant sites was imcomplete and ankylosis at the implant-tooth interface occurred, whereas the height of the alveolar bone was higher than that in the control sites. 3) The width of new periodontal ligament in implant sites was narrower and more irregular than that in the control sites. 4) The anatomical distribution of regenerative nerve in periodontal defects was for the most part demonstrated in both implant sites and control sites, but many regenerative nerve fibers originating from the residual periodontal ligament existed in new bone marrow of implant sites because the pathway of the nerve fibers entering new periodontal ligament was interrupted. For determination of definitive distribution of regenerative nerve and complete bony repair in implant sites, further observation is needed.
