1. Frequency of cerebrovascular disease in the young adult aged below 40 was described. It was found to be approximately 17 per cent of the total cases with cerebrovascular disease in our hospital cases as well as in the autopsy cases. 2. Subarachnoid hemorrhage and occlusive cerebrovascular disease were the types of cerebrovascular disease rather frequently seen in the young adult. 3. Cerebral angiography disclosed vascular malformation more frequently in the juvenile cases with subarachnoid hemorrhage (73.7 per cent) than in those aged over 40 (60.7 per cent). In the former, the frequency of arterio-venous malformation much exceeded to that in aneurysm and in the latter the situation was quite in reverse. 4. Abnormal cardiovascular findings such as hypertension, abnormal ECG, ocular fundi more than KW II and association with diabetes mellitus were minimal in the juvenile cases with non-embolic cerebral infarction. But cerebral blood flow was diminished in many of those cases. 5. Stenotic lesions were more frequently seen in the artery supplying the diseased area of the brain in the juvenile cases than in the elderly. However, number of stenotic lesions irrespective of their location became larger as the age advanced. The significance of these angiographic findings in pathogenesis of cerebral infarction was discussed. 6. The nature of underlying vascular disease should be seriously considered for under-standing the mechanism of stroke particulary in the juvenile cases, which also determine the fate of stroke.
