Symptomatic Vessel Narrowing Caused by Spontaneous Rupture of Craniopharyngioma Cyst : Case Report

頭蓋咽頭腫の嚢胞が自然破裂したことに起因する血管の狭窄から脳梗塞を生じ, 失語と頻眠傾向を呈した36才女性例を報告する.CTにて, 鞍上部に嚢胞性の腫瘍と左側頭頭頂部に脳梗塞巣が認められ, 造影剤により嚢胞壁の他, 左シルビウス裂と前橋槽に増強効果が見られた.血管撮影では左内頚動脈CI部と左中大脳動脈M1部が高度の血管狭窄像を呈していた.両側前頭開頭で腫瘍を亜全摘した.術中, 左内頚動脈および中大脳動脈周囲には乳白色のdebrisが確認された.術後1カ月目の血管撮影では血管狭窄が軽快していた.画像所見と術中所見から, 本症例の血管狭窄は頭蓋咽頭腫の嚢胞の破裂に起因する血管攣縮であった可能性が示唆された.
