Prediction of Effectiveness of Shunting in Patients with Normal Pressure Hydrocephalus by Cerebral Blood Flow Measurement and Computed Tomography Cisternography

正常圧水頭症の疑われる37症例で脳血流量測定と脳槽CTを行い、髄液短絡術の効果を調べた。<SUP>99m</SUP>Tc−HMPAO SPECTを用いて、Patlak plot法により全脳平均血流量を測定した。脳槽CTは以下の4 Typeに分類した。Type I：脳表循環正常。Type II：脳表循環吸収遅延。Type III：脳室内停滞（脳表循環は正常又は軽度遅延）。Type IV：脳室内停滞（脳表循環障害）。転機良好症例の脳血流量は転機不良症例のそれより有意に高かった（p<0.005）。Type Iは髄液短絡術無効だった。Type IIやType IIIの数例は髄液短絡術に反応したが、転機は不良だった。Type IVは髄液短絡術に良く反応し、脳血流量が35ml/100g/min以上の症例は良好な転機が得られた。
