Surgical Treatment of Internal Carotid Artery Anterior Wall Aneurysm with Extravasation During Angiography : Case Report

くも膜下出血を呈した54歳の女性にたいして緊急脳血管撮影を行ったところ, 血管撮影開始後に内頚動脈分岐付近に造影剤のpoolingの出現を発見した. このpoolingは血管外漏出と考えられたが, 形態的には嚢状であり動脈相中期から静脈相後期まで残存しゆっくりと消失した. 術中, 動脈瘤頚を探すために剥離を進めたところ大出血をきたした. 8-0のナイロン糸を使用し血管壁の縫合を行った, 右内頚動脈の血行遮断は約50分であった. 術中, 術後血管撮影の所見より動脈瘤は内頚動脈前壁に存在したと考えられ, 確定診断はできないが仮性動脈瘤の可能性もあり興味ある症例と考えられた.
