Post-troumatic Pituitory Apoplexy : Two Case Reports

頭部外傷を契機に発症した下垂体卒中の2例を基に、その発生原因、治療法について報告する。症例は60才女性、66才男性。いずれも転落事故で、後頭部を強打し同部に線状骨折が見られた。下垂体卒中を起こす前は腫瘍に基づく症状は見られなかった。頭部外傷を契機に視力低下を生じ、両症例とも緊急手術を行った。腫瘍はいずれもdumbell typeで、鞍隔膜で絞扼されていた。発症は外傷により腫瘍の鞍隔膜部にshear strainがかかり腫瘍内出血を起こしたものと考えられる。治療は経蝶形骨洞手術がより侵襲が少なく、視神経に対する減圧には有効と思われる。まず初めは部分摘出でも、視神経の減圧を図り、その後staged surgcryを行って腫瘍を全摘すればよい。
