当院頭蓋底外科チームでは前頭蓋底悪性腫瘍に対してtransbasal and transfacial approachを用いて一塊とした摘出を行ってきた。皮膚切開については頭皮冠状切開に外側鼻切開等を併用するため、術野が2ヵ所以上になり、腫瘍切除時の操作や再建に不都合を生じることがあった。従って、頭皮冠状切開を患側の下顎部まで延長する頭部顔面冠状皮膚切開のみで術野を展開するhemifacial dismasking flapを考案した。本法では顔面皮弁を正中部まで翻転することが可能で、腫瘍切除に際し広範な術野を得ることができ、さらに必要に応じて頚動脈管解放も同一術野で行える。手技的には簡便で、遊離皮弁による死腔再建も一期的に容易に行え顔面に瘢痕を残さない。
