Bilateral Chronic Subdural Hematomas Associated with Intracranial Hypotension : Case Report

Spontaneous Intracranial Hypotension (SIH)に伴う慢性硬膜下血腫例を報告する。現在SIHは軽微な外傷などの後に生じた脊髄のdural lacerarionから脳脊髄液の漏出を来す疾患と孝えられている。症例は56歳の女性で、起立時の頭痛と嘔気を訴え、数日後昏睡状態に陥り搬送された。頭部CTでは両側の慢性硬膜下血腫と各脳槽の狭小化と消失を認めた。数回の再発の度に経皮的血腫吸引術を行ない術後はすみやかな神経症状の改善を認めたが、頭蓋内圧はいずれも低値もしくは険圧であった。SIHに伴う慢性硬膜下血腫は希な病態であり、SIHの臨床像、治療法などについても考察を加えた。
