脳より生きたマンソン孤虫を摘出しえた1症例を経験した。症例は右上肢脱力を主訴とする80歳の男性で、CTでは左頭頂葉に一部不規則な増強効果を伴う広範な低吸収域が認められた。転移性脳腫瘍の術前診断のもとに左頭頂開頭頭を試み、頭中左頭頂葉より生きた虫体を摘出した。術前のMRIでは悪性を思わせる腫瘍性病変の存在にもかかわらず、病側の脳室拡大が認められた。この所見は脳腫瘍と脳マンソン孤虫症との重要な鑑別点になりうると考えた。
