DIAGNOSTIC HYSTEROSCOPY FOR POSTMENOPAUSAL UTERINE BLEEDING

On 428 patients of postmenopausal uterine bleeding, hysteroscopic examination took place before D & C in our hospital. On 154 of them, initial diagnosis was made by conventional examination methods in another clinic, but on the remaining 274 patients the initial common examinations could find no lesion causing immediate uterine bleeding. Hysteroscopy can easily get a clear view of intrauterine lesions in senile women, i.e., since the uterus has an atrophic wall and is dilatable, it reveals more abnormalities. Hysteroscopy could not overlook even subtle pathology such as limited lesions of endometrial carcinoma and localized masses of polyp or submucous myoma. Moreover, hysteroscopy revised the initial diagnosis of pyometra, endometrial hyperplasia and ovarian tumor in some instances into final diagnosis of entirely different pathology such as endometrial carcinoma or polyp. Eventually fifty instances had no different findings on hysteroscopy from those of the normal senile uterus. At the time of hysteroscopic examination, the lesion might have been already removed or cured by previous examination. Hysteroscopy has been proved an indispensable diagnostic method in patients with postmenopausal uterine bleeding.
