[SHORT REPORTS]Effects of Dimethyl Amiloride on Ionic and Metabolic Alterations of Isolated Rabbit Hearts under Ischemia-Reperfusion in the Absence of Preischemic Cardioplegia

Protective action of dimethyl amiloride (DMA), a specific inhibitor of sarcolemmal Na^+/H^+ exchange, against ischemia-reperfusion injury was investigated in rabbit hearts perfused in vitro. The hearts were subjected to normothermic (37℃) global ischemia for 30 min followed by reperfusion for 30 min. DMA (10μM) was added to the preischemic perfusate for 2 min. Treatment with DMA improved recovery in the left ventricular developed pressure at 30 min of reperfusion form 78±2.7% (control) to 99±2.8%(p<0.05). Sequential changes in intracellular hydrogen and high energy phosphates during ischemia-reperfusion were monitored using ^3^1P unclear magnetic resonance (NMR) spectroscopy. Unlike our previous experiments using preischemic cardioplegia, ^1^) neither intracellular pH (pHi) nor high energy phosphates were affected by the DMA treatment during ischemia-reperfusion. The discrepancy and possible mechanisms of DMA to ameliorate ischemia-reperfusion injury are discussed.
