The heat irradiated by moxibustion produces local dermal vasodilation, which increases vascular permeability thus promoting local metabolism. Moxibustion therapy has been applied to various disorders including immune diseases in oriental medicine. We examined moxibustion effects on substance P (SP), heat shock protein (Hsp70) and c-Fos distributions in the mouse spinal cord immunohistochemically. Firstly, moxibustion was performed on the dorsum at L3 level. SP immunoreactive neurons were observed in laminae I and II of the spinal cord after moxibustion. Hsp70 positive neurons were also observed in the ventral horn. Secondly, we compared the moxibustion effects with those produced by other noxious stimuli. Moxibustion and ordinary heat stimulation was applied cutaneously on the plantar surface of the right hindpaw. Formalin and histamine was injected subcutaneously into the plantar surface of the right hindpaw. Moxibustion and histamine stimulation induced SP-immunoreactivity in laminae I and II, thermal stimulation produced the strongest reaction in lamina I, while formalin stimulation reduced SP positivity by 2 hours. c-Fos immunoreactive neurons were observed in lamina I after two hours of moxibustion and formalin stimulation. Hsp70 positive neurons were observed both in the dorsal and ventral horns after moxibustion, and histamine and heat stimulation. The immunoreactivity was not observed after formalin injection. To sum up, distributions of the immunoreactive neurons after moxibustion and other noxious stimuli were different in loci and in intensities. Thirdly, moxibustion effects on immunoreactivity produced by formalin stimulation were examined. When moxibustion was performed before or after formalin stimulation, distributions of c-Fos positive neurons were different from those following formalin stimulation alone. The suppression effect of moxibustion on c-Fos expression coincides with pain suppression and the easing of stiffness as a general therapeutic effect.
