Detailed psychiatric evaluations were made of 308 patients admitted to the Critical Care Medical Center (CCMC), Kinki University School of Medicine after suicide attempts between January, 1983 and December, 1996. Also included are 5 cases who received psychiatric treatment by the author during the one year when the author worked regularly at the CCMC. Cases are described to show concrete examples of the course of suicide attempt. From this study, the following 7 points were revealed. The number of suicide attempt cases admitted to the CCMC has increased each year. Suicide attempts were committed frequently during psychiatric treatment by patients with endogenous psychosis, but suicide attempts by patients with neurotic disorders were more often prior to psychiatric treatment. Patients with endogenous psychosis tended to choose more physically invasive attempts, but those with neurotic disorders tended to choose less physically invasive attempts. However, among patients with neurotic disorders, extreme physically invasive attempts were often taken by patients with borderline personality disorders. Jumping in front of trains or vehicles was different from jumping from a height in its psychopathological background, although both were physically invasive attempts. Suicide attempts using medical preparations, particularly psychotropic drugs, are on the increase, and precautions are needed when prescribing such drugs. There was one group of patients with a high survival rate among those with schizophrenia who repeatedly attempted suicide. It is necessary to pay attention to the patients with borderline personality disorder before discharge, especially those who are allowed to stay home. Patients who have attempted suicide should be handled on the basis of the findings in this study. Repetition of suicide attempts is considered preventable.
