片麻痺幼児に対する作業療法の一考察

A male child with right hemiplegia, who was diagnosed with congenital hydrocephalus and epilepsy, was referred to Occupational Therapy (OT)and OT services were provided for him for 4 years and 4 months (from the age of 19 months to 5 years and 10 months). His OT progress was divided into four separate terms following his developmental change. In the first term, the principal objective of OT was to establish a stable relationship between the child and the occupational therapist. In the second term it was to promote his gross motor development. Third term OT serevices promoted his cognitive development ; and in the last term the occupational therapist tried to reduce his problem behaviors. It was concluded that in OT for a child with hemiplegia the following are important : 1) bimanual activities during play are effective in the treatment of the affected side's upper extremity, and 2) occupational therapists have to pay attention to the child's problem behaviors. It was also concluded that the mother's education include explanation of the disease and the gradual provision or specific guidance regarding care of the child at home, and not to criticise the mother's care of the child.
