Prostatic Cancer in Japan : Predominancy of Advanced Patients

To know the characteristics of prostatic cancers in Japan, I examined 1037 Japanese cases especially on their clinical stages. The age distribution of the patients with prostatic cancer was highest at around 70 years old, which was approximately the same to that in Western countries. Most cases in Japan, however, were in the advanced stages when the diagnosis was first made, while in the United States and European countries, the patients with prostatic cancer were evenly distributed in each stage. I examined a relation between the clinical stages and morphologic parameters of malignancy using glandular differentiation grading and Gleason's grading of malignancy. Each of Gleason's grading, histological differentiation, and nuclear atypia of the cancer cells was well correlated with death-rate of the patients, and considered to be a reliable parameter for predicting the prognosis. Early detection system in Japan may be the most considerable factor to explain the predominance of advanced cases in Japanese prostatic cancer.
