Improved Nucleic Acid Precursor Incorporation Assay for Chemosensitivity Testing of Bladder Cancer

Nucleic acid metabolism was investigated to know the metabolic activities of bladder carcinoma cells cultured in vitro. Radioactive nucleic acid precursors, <14>^C - Formate which is incorporated into nucleic acid via de novo synthetic pathway, and <14>^C - Adenine via salvage pathway, were used. <14>^C - Formate incorporation was much higher in grades 2 and 3 of bladder carcinoma cells than in the normal epithelium of the bladder. Also it was highest in stage B 2 bladder carcinoma cells. As for <14>^C - Adenine it was found that the highter the grades and stages of the cells proceeded, the higher incorporation rates were demonstrated. To select the most suitable anti - cancer agents for the surgical adjuvant chemotherapy, the chemosensitivity test using the results of these nucleic acid metabolism was developed. The principle of this test was the inhibitory activities of anti - cancer agents on the incorporation. The method was applied to the 36 patients who underwent total cystectomy. Mitomycin C and Adriamycin were the most potent agents used in the chemosensitivity test. Mitomycin C was used for the post - operative adjuvant chemotherapy. Mitomycin C - sensitive patients showed superior results on the local recurrences and metastases to the patients with Mitomycin C - insensitive patients. From these results, the chemosensitivity test was considered useful for the selection of suitable anti - cancer agents for the surgical adjuvant chemotherapy.
