Heart rate trend pattern was investigated in 3 healthy subjects and 3 cases with pheochromocytoma to clarify the correlation of the heart rate trend with hypersympathetic state. In 3 cases with pheochromocytoma, both Holter ECG and catecholamine were estimated before, immediately and 5 to 9 years after operation. The results were as follows; 1) One of 3 cases with pheochromocytoma showed a peculiar, sawlike pattern in the heart rate trend and the other cases revealed a spikelike pattern in the trend, although none of them was shown in the healthy subjects. These abnormal patterns in 3 cases with pheochromocytoma disappeared immediately after operation. 2) Left ventricular voltage (SV1+RV5) was decreased in 2 of 3 cases after operation, although Q-T interval showed no change after operation in all the three cases. 3) Noradrenaline and adrenaline in both urine and blood showed abnormally high level before operation and decreased to normal range after operation. These results suggest that the peculiar heart rate trend patterns reflect the hypersympathetic tone and the heart rate trend pattern by Holter ECG is useful for assessment of autonomic nerve function.
