Newborn twins are identical in terms of their exposure risk for maternal-infant (vertical) transmission of infectious agents, but differ with respect to their risk of infection via horizontal transmission. This report describes a theoretico-epidemiological method to estimate the prevalence of maternal-infant and horizontal transmission of hepatitis B virus, using serological prevalance of hepatitis B markers among twin subjects. Hepatitis B virus markers (anti-HBc and anti-HBs) were determined in each of 144 pairs of twins. Serological evidence of previous infection of hepatitis B virus was present in 33 both-positive cases and 48 either-positive cases. The prevalence of maternal-infant and horizontal transmission was estimated to be 16.6% and 23.0%, respectively. The group born prior to 1919 had horizontal infection in most cases, which indicated the spread of infection at that time, and the increase in maternal-infant infection in subseqent generations was noted. The twins of hepatitis B virus both-positive cases tended to show a high γ-GTP over 50IU/L. The mode of infection seemed to affect liver function for a long period.
