To investigate the source of astigmatism seen in hard contact lens wearers, changes in dioptors of astigmatism were measured using an autorefractometer in 158 eyes (84 phakic, 34 aphakic, 40 pseudophakic) wearing hard contact lenses. Regardless of whether the eye was phakic, aphakic, or pseudophakic, astigmatism as high as 0.75 dioptors was most frequently observed in hard contact lens wearers. These results suggest that the astigmatism seen in hard contact lens wearers may be induced by wearing of hard contact lenses. To test a hypothesis that vertical movement for alignment of hard contact lenses on the cornea might induce astigmatism, an attempt was made to compare values of the measured astigmatism caused by wearing a hard contact lens with those calculated by a simulation model of the eye based on the physica1 apposition of a hard contact lens and the cornea. The power and axis of the calculated astigmatism were very close to those of actually determined astigmatisms. This suggests that astigmatism seen in hard contact lens wearers is generated by the movement of hard contact lenses following the contact lens being out of alignment.
