To develop a new treatment for hepatic carcinoma, cis-diamminedichloroplatinum II (CDDP) suspended in an oily contrast medium, lipiodol (LPD), was slowly administered via the gastroduodenal and the proper hepatic artery into rats with 3'-methyl-4-dimethylaminoazobenzene (3'-Me-DAB) induced hepatic carcinoma. Based upon the results of a preliminary experiment using normal rats, the volume was set at 0.1 ml per rat, (10 mg of finely ground CDDP suspended in 1 ml of LPD). Compared to other groups given CDDP+saline, LPD alone or saline alone, tumor growth rates in the CDDP+LPD suspension group were lower 5 days after administration and even tended to be lower at the 21st day. Low kilovoltage radiography 5 days after administration showed selective CDDP+LPD accumulation in the carcinoma. Platinum concentrations in the liver was significantly higher in the carcinoma than in other, non-tumorous areas. Histologically, carcinoma cells in the center of the carcinoma nests were moderately degenerated and necrotic. Necrosis rates in the CDDP+LPD suspension group were 20.1% 5 days after administration and 40.2% on the 21st day, both values being significantly higher than in the other groups. No degeneration nor necrosis was found in the non-tumorous area of the liver. Body weight in the CDDP+LPD suspension group decreased 5 days following administration but some of the animals had recovered to their original weights by the 21st day. These results show that hepatic arterial administration of CDDP+LPD suspension is effective in the treatment of hepatic carcinoma.
