To test the diagnostic value of the flicker electroretinogram (ERG) in diabetic retinopathy, both flash and 30 Hz flicker ERGs were investigated using a new developed LED light sourceelectrode contact lens. The control study made on 110 normal eyes revealed that the peak latency of 30 Hz flicker ERG (30 Hz peak latency) was less than 21.0 msec in 106 (97.3%) eyes. An age-dependant prolongation of the peak latency was also recognized. The prolongation of the 30 Hz peak latency was remarkable in diabetic retinopathy ; in 43 eyes of diabetic retinopathy examined, 22 (51.1%) eyes showed their reak latencies of less than 21.0 msec. In all cases of diabetic retinopathy, the 30 Hz peak latency was then compared with the O_1 peak latency of the oscillatory potential of the flash ERG. They were well correlated to each other with a correlation coefficient of 0.785,which suggests that the study of the 30 Hz peak latency is diagnostically significant for the electrophysiological assessment of retinal function in diabetic retinopathy. When diabetic retinopathy was classified into 12 sub-stages according to Fukuda's classification, the 30 Hz peak latency was slightly prolonged in the initial non-proliferative stages such as AI and AII, but it was far out of the normal range in the proliferative stages such as BIII and BIV. For the stages of BI and BII, which are clinically the most appropriate stages for photocoagulation, a compensatory retinal hyperfunction was suggested since the 30 Hz peak latency stayed within the normal range even when it was compared with that of the initial stages of diabetic retinopathy such as AI and AII. In conclusion, the flicker ERG may be useful not only for electrophysiological assessment of retinal function in various stages of diabetic retinopathy but also for timing of photocoagulation.
