The pathogenesis of cortical damage of the contralateral hemisphere in experimental cerebral ischemia was evaluated. Unilateral carotid arteries were clipped for 1-hour in adult Mongolian gerbils. Regional cerebral blood flow was measured with a H_2 gas clearance method. The degree of tissue damage was evaluated by a "vacuolic index" calculated from morphometric procedures. The cortical tissue damage of the contralateral hemisphere was qualitatively similar with that of the ischemic side. The degree of tissue damage of the opposite hemisphere did not correlate with the hemodynamics, but did with the ischemic side to a certain extent. It was, therefore, considered that some factors other than hemodynamics seem to play an important role in the pathogenesis of contralateral cortical damage, and the identity of these possible factors is discussed.
