Only a few noninvasive methods of assessing pulmonary regurgitation are available. Using a combination of pulsed Doppler and cineangiography or color Doppler, pulmonary regurgitation was estimated in 47 chidren after the tetralogy or Fallot was repaired. In patients with a pressure gradient of <15 mmHg across the pulmonary valve, the ratio of diastolic to systolic mean cross-sectional areas in the main pulmonary artery on the right ventriculogram was lower in six patients with 3+ or 4+ pulmonary regurgitation on the pulmonary arteriogram than in five patients with 0 to 2+ pulmonary regurgitation (P<0.05). in seven patients with a pressure gradient of <10 mmHg, the pulmonary regurgitant fraction was calculated from the product of the time-velocity integral by pulsed Doppler times the mean cross-sectional area of the main pulmonary artery. The pulmonary regurgitation area index (the maximal pulmonary regurgitation area/body surface area) on color Doppler was calculated using a track ball. The pulmonary regurgitation area index correlated well with the regurgitant fraction by pulsed Doppler (r=0.80). A pulmonary regurgitation area index of ≩4.0 was approximately equivalent to a pulmonary regurgitation of 3+ or more. In 25 patients with a pulmonary regurgitation of 3+, the pulmonary regurgitation area index was not influenced by age, postoperative period, mean main pulmonary artery pressure or right ventricular pressure. Evaluation of pulmonary regurgitation after repair of the tetralogy of Fallot using color Doppler is useful, because it can be performed easily and repeatedly, regardless of the presence of residual pulmonary stenosis.
