While rhinoplasty is an important issue among otorhinolaryngological societies in the United States, Europe, and Korea, Japanese otorhinolaryngologists remain unfamiliar with external nose surgery, and rhinoplasty is mainly done by plastic surgeons. Aesthetic rhinoplasty may be done without considering nasal functions. We report a case of nasal deformity successfully treated by revision open septorhinoplasty and discuss the role of otorhinolaryngologists in rhinoplasty.A 63-year-old man with an external nasal deformity causing progressive nasal stiffness had undergone septoplasty for nasal septum deflection elsewhere 30 years earlier. His nose was contracted caudally with nasal valve collapse. The nose showed septal perforation and deflection. In open septorhinoplasty under general anesthesia, the deformity was corrected and nasal stiffness markedly improved postoperatively. No complication was observed, and the surgery improved the man's quality of life.Rhinoplasty includes septorhinoplasty, dorsal augmentation, deviated nose correction by osteotomy, hump reduction, and tip surgery. These techniques are useful in surgery for patients with nasal stiffness. Rhinoplasty, both functional and aesthetic should thus be done by rhinologists who take considerations into nasal function.
