女性ホルモン補充療法中に脳梗塞を発症した3症例

要旨：2007年10月から2012年3月までの4年半の期間に女性ホルモン補充療法中に脳梗塞を発症した3症例を経験した．3例中2例は更年期症候群の症状緩和目的で，1例は骨粗鬆症改善目的であった．3例とも女性ホルモンタイプは結合型エストロゲンと酢酸メドロキシプロゲステロンの合剤であった．3例中2例に高血圧と脂質異常症，1例に喫煙の危険因子を持っていた．3例とも内頸動脈に粥状硬化，プラーク，狭窄を認めず，弁膜症，心房細動，動脈解離もなかった．いずれも5年以上内服，脳梗塞は軽症であった．女性の脳梗塞の場合は女性ホルモン補充療法有無の確認が必要とともにすでに投与された場合はその中止も検討すべきである．
