長崎県離島発症の急性期脳梗塞患者に対するrt-PA投与～ヘリコプター搬送による"drip-and-ship"法の試み～

【背景】Recombinant tissue plasminogen activator（rt-PA）静注療法の適応は，全脳梗塞患者の約5％以下である．僻地においてはその適応はより小さいものと推測される．【目的および方法】2007年1月から2011年6月までの間，長崎県離島にて発症した急性期脳梗塞患者で当院へヘリコプター（ヘリ）搬送となり入院となった患者をretrospectiveに再評価し，rt-PA"drip-and-ship"法の可能性および課題について再検討した．【結果】4例に対して"drip-and-ship"法が施行された．2例において閉塞血管の再開通が得られ症状の改善が見られた．投与後のヘリ搬送に伴う症候性頭蓋内出血例はなかった．【結論】離島においては"drip-and-ship"法が安全であり理想的であると考えられるが更なる症例の集積が必要である．
