くも膜下出血にて発症したＡ１部非外傷性解離性前大脳動脈瘤の治療経験とその現況

くも膜下出血で発症したA1部の非外傷性解離性前大脳動脈瘤の1症例を経験した．症例は51歳男性で，突然の意識消失で発症し入院した．頭部CT，頭部MRI/MRA，脳血管撮影から左前大脳動脈A1部の解離性動脈瘤の破裂によるくも膜下出血と診断した．このため，左前頭側頭開頭による動脈瘤trapping術を施行し，良好な転帰を得た．この治療経験から，近年の文献的考察を加え，A1部の解離性前大脳動脈瘤における出血例の最近の治療を検討すると，治療は手術による動脈瘤trapping術をベースとして他の術式を検討することが良いと思われた．
