Recombinant tissue plasminogen activator (rt-PA)静注療法によりいったん症状が改善した後，24時間以内に症状の再増悪を来し，急性期頸動脈ステント留置術を施行した頸動脈狭窄症2例を報告する．特に主幹動脈狭窄に起因する脳梗塞の場合，rt-PA静注療法後の再梗塞の危険性は高く，その予防や再増悪した際の治療法の確立が必要である．
