【目的】地方病院でのrt-PA静注療法の現状を明らかにすることを目的とした．【方法】rt-PA静注療法を導入した2007年7月から2009年7月までの25カ月間に経験した脳梗塞の症例を対象とし，rt-PA静注療法を行った11例について検討した．【結果】11例のうち1例を除き治療効果を得ることができ，重大な合併症は認めなかった．軽症のためrt-PAを投与しない例の中に症状が悪化するものがあった．【結論】適正治療指針に従ってrt-PA静注療法を行うことにより，良好な成績を得ることができたが，当初軽症でrt-PAの適応なしと判断した例で後に症状悪化をみた例があり，軽症例に対する適応に関して検討の余地があると考えられた．
