脳幹梗塞発症急性期に痙攣発作を認めた１例

脳幹梗塞発症急性期に痙攣発作を呈した72歳の男性症例を経験した．来院時に意識障害と左上下肢麻痺を認め，来院直後から痙攣が出現した．頭部MRI拡散強調にて橋の右側腹側と両側背側に梗塞巣が描出され，MRAで脳底動脈に狭窄所見を認めた．数日の経過で痙攣発作の出現はなくなったが，右眼球の内転障害が残存した．大脳皮質に痙攣の原因となり得る梗塞巣は認めなかった．脳幹虚血にともなう痙攣発作は稀な徴候ではあるが，脳幹梗塞にともなう痙攣発作の可能性があることも考慮する必要性が示唆された．
