左手自己所属感の消失のみを呈した脳梁離断症候群の１梗塞例

40歳，右利き女性．右側後大動脈領域の脳梗塞を発症し，脳梁体部から膨大部右側にも病変を認めた．左同名半盲に加え，「左手が自分の手のような感じがしない」という訴えがあった．左手の観念運動失行や拮抗失行はなかった．他人の手徴候（alien hand sign）については定義の変遷があり，表現が混乱しているが，Brionによる本来の定義では「脳梁損傷患者が視覚外で左手を右手でつかんだときに感覚障害がないのに自分の手と認知できないこと」1）である．本例はこのBrionの定義する他人の手徴候にあてはまると思われ，脳梁離断による症状の可能性が示唆された．
