未破裂脳動脈瘤を虚血性脳血管障害急性期のｒｔ‐ＰＡ静注療法の禁忌とすることは妥当か

脳梗塞急性期のrt-PA静注療法に用いるアルテプラーゼの添付文書には禁忌の項目に「頭蓋内出血の既往又は頭蓋内腫瘍、動静脈奇形、動脈瘤などの出血性素因のある患者」という文章が記載されており、未破裂脳動脈瘤を保有する患者の治療が問題となる。この文章がどのような経緯で添付文書に現われたか、その内容に科学的な根拠があるか、また現実に治療中に脳動脈瘤破裂を来した例がどのくらいあるか、などを検証した。結論として未破裂脳動脈瘤を脳梗塞急性期のrt-PA静注療法の対象外とする科学的根拠は発見できなかった。
