突然発症の片麻痺に対してアルテプラーゼ静注療法を検討した急性脊髄硬膜外血腫の１例

症例は74歳、女性。左後頚部痛を伴う左上下肢麻痺の発症後1時間17分で来院した。虚血性脳血管障害を疑い、アルテプラーゼ静注療法を検討したが、麻痺が急速に改善したため適応外と判断した。第2病日にかけて麻痺はさらに改善したが、左ホルネル症候群が明らかとなり、脊髄MRIで脊髄硬膜外血腫と診断した。突然発症する顔面を含まない片麻痺症例では、脊髄硬膜外血腫などの頚髄病変を鑑別すべきであり、本症例では同側のホルネル症候群が重要な所見であった。
