Two cases of spheno-ethmoidal mucoceles with the bony destruction of the skull base are presented. It is sometimes difficult to differentiate paranasal tumors and cystic lesions only by radiological measures especially when these lesions are confined to posterior ethmoid or shpenoid, accompanying bone destruction. In such cases, past history of nasal or paranasal surgery is of help in suspecting mucocele and in addition, exploratory puncture often aids in establishing the correct diagnosis. However, one should be very careful in performing the puncture of the cysts adjacent to the skull base with bony erosion, since the needle may accidentally puncture the dura, causing intracranial hemorrhage.
