鼻出血を主訴とするOSLER病の手術療法と鼻粘膜血管の微細構造

A 58-year-old woman with hereditary hemorrhagic telangiectasia recalled frequent nosebleeds since about 30 years ago. She had an surgery of submucous resection of the nasal septum through the incision in the mucosa of the lip under general anesthesia and she bleeds only one time during 6-months. Biopsy specimens of typical lesions of septum were taken for electron microscopy during surgery.<BR>Endothelial cells of the dilated venule were thin and some of them were degenerated and rent. The basement membrane of the venule was ruptured and reduplicated. Pericyte of venule did not form a continuous coat in these vessels and was poor of myofilament. Perivascular tissue was composed of a few collagen fiber and many vacuoles. Connective tissue distant from the venule was almost same with normal one. The tunica media of small artery was thin and poor of myofilament.
