Two cases of pseudosarcomatous carcinoma of the larynx are presented. The lesions are polypoid in one and sessile in the other. The tumors microscopically manifest sarcoma-like areas with squamous cell carcinoma, the sarcoma-like component being apparently infiltrative spindled squamous cell carcinoma. The neoplastic spindle cells do not appear to be truly anaplastic, but represent loss of cohesiveness of the basal or prickle cell layer of the squamous cell carcinoma which is usually superficial or occupies a small portion of the tumor. A marked proliferation of granulation tissue is also associated with the infiltrating spindle cell carcinoma.<BE>The origin and relationship of the two types of tissue, the sarcoma-ike and the squamous cell, in this lesion have not been clearly established. However, the findings in the present cases support the view that the lesion represents a unique variety of squamous cell carcinoma.<BR>Clinically, it is generally agreed that the tumors do not metastasize and afford the more favorable outlook than ordinary squamous cell carcinoma of the larynx. If one is aware enough of this tumor, more conservative treatments may be applicable to it under certain circumstances.
