A 12-year-old boy who was diagnosed to have aplastic anemia at 6 years old underwent allogeneic bone marrow transplantation. He was conditioned with high dose cyclophosphamide and limited field irradiation (eyeballs, lungs and testes were shielded). His post-BMT course was complicated by chronic graft-versushost disease, which resolved four years after BMT. His post-transplant evaluation on gonadal function, using LH-RH provocation test and testis biopsy at 13 years old (1 year after BMT), revealed marked hypogonadism. He fathered a normal child seven years after BMT, and reevaluation showed normalized gonadal function. His paternity was proved by HLA typing of the child and the wife.
