Clinical and morphological heterogeneity of acute promyelocytic leukemia.

Five cases of acute promyelocytic leukemia (APL) were encountered among 60 children with acute non-lymphocytic leukemia. These cases were clinically and morphologically heterogenous. Clinical and laboratory findings of disseminated intravascular coagulation were observed in 3 of these cases. Two cases had features compatible with a diagnosis of M3-variant. In 2 other cases, the leukemic cells had a lobulated nucleus and numerous azurophilic granules, but no Auer bodies were detected. In one of the latter cases, electron microscopy revealed typical but small Auer bodies and stellate rough endoplasmic reticulum, which are characteristic of APL. Only one case exhibited classical features typical of APL. The ultrastructural distribution of peroxidase reaction products indicated differences in the stages of maturation arrest of promyelocytes among the cases.
