We report a rare case of polycythemia vera in a 14-year-old girl. She had marked splenomegaly and her peripheral blood values were as follows : hemoglobin 16.6 g/dl, red blood cells 815 × 104/μl, white bloodcells 15, 700/μl, platelets 13.7 × 104/μl. Erythropoietin was low, and spontaneous colony formation and erythropoietin-hypersensitivity of erythroid progenitor cells were observed. JAK2 V617F mutation was absent. Phlebotomy was per-formed initially. After the beginning of interferon therapy at a dose of 3 × 106 U 3 times per week intramuscularly, the hemoglobin value was normalized without phlebotomy, and splenomegaly decreased in size with interferon therapy 6 times per week. She has been well controlled with interferon therapy for 13 months. Long-term follow-up is necessary in order to detect the complications or leukemic transformation.
