A Case of Acute Promyelocytic Leukemia with All-Trans Retinoic Syndrome

A 6-year-old boy with acute promyelocytic leukemia (APL) was treated with all-trans retinoic acid (ATRA) and developed retinoic acid syndrome (RAS). On the first day with the treatment of ATRA, pain on the bilateral dorsum pedis appeared. On the 5th day, the white blood cell count was elevated to 41, 000/μl and the sign of weight gain, tachypnea and pulmonary infiltration, cardiac enlargement on the chest X-P were noted, so Retinoic acid syndrome (RAS) was diagnosed. He was improved by prompt administration of dexamethazone, and discontinuation of ATRA, combined with chemotherapy. The respiratory sign of RAS can be early recognized by physical findings and monitoring oxygen saturation of arterial blood by pulse oximetry. It is needed that a standard therapy for RAS with ATRA be established.
