Nine patients with childhood MDS received allogeneic stem cell transplantation (SCT). One patient had RA, 3 had RAEB (± T), 2 had infantile monosomy 7 syndrome and 3 had JCML. The RA patient received SCT after a standard risk regimen of BU and CY, and has been disease-free for more than 4 years. A RAEB child with infection died soon after a conditioning regimen of TBI, VP-16 and CA followed by SCT. The other 2 RAEB (± T) patients were treated by low-dose CA and ANLL protocol, and one has achieved PR. Four of the 5 patients with monocytosis received OCLSG N91 protocol, and 2 have achieved CR. Among the 7 patients who received SCT after a high-risk regimen of TBI, Thiotepa and L-PAM, 4 patients continue CR. This high-risk regimen seems to be useful for advanced childhood MDS, especially for JCML.
