Recurrent Pancreatitis Induced by Vincristine in a Patient with Acute Lymphoblastic Leukemia.

We report a case in which a four-year-old girl with acute lymphoblastic leukemia (ALL) experienced recurrent pancreatitis induced by vincristine. At the end of the remission-induction therapy, she suffered from severe acute pancreatitis probably due to L-asparaginase. Fever and abdominal pain recurred when the therapy for leukemia was restarted without L-asparaginase after improvement of pancreatitis, and finally a pancreatic pseudocyst with intracystic hemorrhage was presented. She suffered from fever and pain when administered vincristine only. We considered that vincristine was the factor that worsened the pancreatitis. We reduced the dose of vincristine and then gradually increased it again, reaching the scheduled dose 16 months after the onset of pancreatitis. Vincristine-induced pancreatitis is rarely reported, but precautions should be taken to prevent vincristine-induced pancreatitis in leukemia patients, especially with a history of pancreatitis.
