We report a case in which a girl with non-Hodgkin's lymphoma (NHL) developed avascular necrosis of the bone about two years after receiving an autologous bone marrow transplantation. A 15-year-old girl with swelling in the right neck was diagnosed as having NHL, stage II-A, lymphoblastic-type based on laboratory findings. After chemotherapy, remission was achieved, but six months later, she experienced a local relapse around the tract of the inner ear. Because of a residual tumor after receiving radiation therapy, we performed an autologous bone marrow transplant (ABMT) with a preconditioning regimen of Thiotepa and L-PAM. Her post-transplant course was uneventful except for bacterial sepsis. She complained of left lower leg pain and walk disturbance at 19 months post-ABMT. Radiological imaging such as MRI and bone scans enabled us to diagnose avascular necrosis of the hip and left tibia. With conservative therapy, her clinical symptoms disappear-ed and no progression of AVN was observed by MRI. Although AVN is not a rare complication in English literature, to our knowledge, this is the first report in which AVN was observed after ABMT in Japan. In view of the progress of treatments for pediatric cancer and the increasing longevity of survivors, one needs to pay much attention to this type of complication.
