Diagnosis and Treatment of Cytomegalovirus Infection in a Four-Year-Old Boywith Acute Lymphocytic Leukemia.

We report here on cytomegalovirus (CMV) infection diagnosed in a four-year-old boy with acute lymphocytic leukemia (ALL) 16 months after the initiation of chemotherapy for ALL. For early diagnosis of CMV infection, altered lymphocyte subsets (elevation of activated T-cells, and reduction of 4/8 ratio and NK-cell population) as well as the elevation of serum ferritin, 2-5AS activity, LDH, CRP, serum and urinary β<SUB>2</SUB> microglobulin, and cold hemagglutinin were found to be useful before virological confirmation. Unresponsive to treatment with ganciclovir, hyperimmune γ-globulin and acyclovir, the disease progressed and the patient subsequently developed severe retinitis six months after the onset of the CMV infection. Chemotherapy for ALL was stopped at 28 months after the onset, and the retinitis improved markedly. However, the boy experienced a relapse of ALL nine months later.
