A Case of Hemophagocytic Syndrome Associated with Rubella and EBV Dual-Infection Successfully Treated with High-Dose Gamma-Globulin.

A 4-year-old girl with hemophagocytic syndrome (HPS) associated with rubella and EBV dual-infection is described. On admission, she was diagnosed as having rubella-associated HPS because of persistent fever after rubella infection, but laboratory examination revealed that she was seropositive for EBV-IgM as well as for rubella-IgM. The fact that she suffered from both rubella and EBV simultaneously could have been related to the pathogenesis of her HPS. She was treated with prednisolone and then received high-dose gamma-globulin therapy, the latter was effective. Serial studies of IFN-γ, IL-6 and s-IL2R were perfomed in order to determine the therapeutic mechanism of action of high-dose gamma-globulin therapy. IFN-γ levels decreased from 45.4 U/ ml before to 5.7 U/ml after high-dose gamma-globulin therapy.
