A Study of the Automation of Cytodiagnosis (The Second Report):Cytophotometry Using Photomicroscanner and a Trial of Data-processing

In the previous report the Automatic Cytoscreener was described. It has the function of recognizing a nucleus as a single nucleus and measuring and processing the nuclear area. All the steps including the mechanical operation are completely automated. However the Automatic Cytoscreener has a deficiency that the cytodiagnostic criterion adopted is only one, that is, the area of nucleus. In this report an attempt is made to add the function of measuring optical density to the Automatic Cytoscreener. As a preliminary experiment a prototype apparatus called "Photomicroscanner" has been constructed. The apparatus has functions to obtain several cytophotometric parameters based on the geometry and densitometry of a cell. The area and the total optical density of a cell or a nucleus are calculated automatically and are displayed by the digital display unit, and the scanned images are described on an XY recorder geographically. Eight cytodiagnostic parameters : the area of a cell, the area of a nucleus, the nuclear cytoplasmic ratio, the total optical density of cytoplasm, the total optical density of a nucleus, the average optical density of cytoplasm, the average optical density of a nucleus, and the nuclear cytoplasmic optical density ratio were calculated. None of the above-mentioned parameters was found to be sensitive or specific to detect cancer cells. Then criteria for diagnosing cancer cells were detected by combining these eight parameters. When plural parameters were combined by the logical products, considerable number of combined criteria were found to be sensitive or specific. As a trial a branch logic was tentatively established and memorized in a digital computer. Computer-processed results were satisfactory in the internal sample but less satisfactory in the external sample. Trial and error are to be repeated to establish a cytodiagnostic criterion with high sensitivity and specificity.
