We have performed surgical reduction and fixation by endoscopic sinus surgery for blowout fracture of the medial orbital wall in two cases. Both cases complained of diplopia before operation. On CT, blowing out of the orbital contents into the ethomoidal sinuses was suspected. Since the symptom was not improved by the conservative treatment at other hospitals, the patients referred to our hospital. Operations performed two weeks and four weeks after injury, then diplopia improved. Endoscopic sinus surgery for blowout fracture of the medial orbital wall is minimally invasive and excels aesthetically. However, differentiating ethomodial lamellae, fracture fragments, edematous mucosa, clot, orbital contents and dealing with them properly are considered essential in the operative procedure.
