The use of navigation system assisted surgery provides numerous benefits when treating temporal bone lesions, because lateral skull lesions tend to consist of firm bony structures, and the accuracy of registration is usually maintained until the end of the operation. Although the margin of error with this system was 1 mm or so, this was still sufficient to be used as an effective operational map. We experienced 3 cases of outer ear cancer, and in the third case, although the range of pathology was unclear on the thin sliced HRCT image, it was clearly visualized by 3 Tesla MRI (3T-MRI). We decided to fuse both HRCT and 3T-MRI images to clarify the location of the tumor by using iPlan ENT®. Using these procedures, we traced the border of the tumor and then made a 3-dimensional view. In these simulations, we also made images of important surgical points under control. We tend to rely more on the HRCT findings in temporal bone surgery, and so we choose the image of HRCT as the view of perooperative navigation system. We herein emphasize the efficacy of performing preoperative simulations using iPlan ENT®.
